Registration Form

Register a new student or renew for the new school year in about ten minutes. Every new student starts with a trial class or audition so we can place them at the right level.

How to register

Four steps from first visit to first class.

  1. Trial class or audition

    All new students attend a trial class or audition for level placement. Book one.

  2. Complete this form

    Student and parent details, programs of interest, and the school’s policy agreements.

  3. Pay at the front desk

    The $75 annual registration fee and your first tuition payment, by cash, check or Zelle.

  4. Start classes

    We confirm your class times. See the weekly schedule.

Returning students: please submit an updated registration and policy statement every year, before the start of academic-year and summer classes.

Enrollment & liability

I hereby enroll myself (my child) in Shostakovich School of Music, Art and Dance and agree to pay full tuition and fees. I agree that I will not hold the school or any member of the faculty or employees liable for injuries sustained on or off its premises, or illness of any kind contracted by my child (or me) while a student of Shostakovich School of Music, Art and Dance.

Waiver of responsibility

  1. Assumption of risk: I understand that participating in school activities involves certain risks, including but not limited to physical injury. I voluntarily assume all such risks on behalf of my child.
  2. Release and waiver: I release Shostakovich School of Music, Art and Dance, its instructors, staff and volunteers from any and all liability, claims, demands, actions or causes of action arising out of or related to any loss, damage or injury that may be sustained by my child while participating in activities at the school.
  3. Medical emergencies: In a medical emergency, I authorize the school to seek necessary medical treatment for my child. The school will make reasonable efforts to contact me as soon as possible.
  4. Personal property: The school is not responsible for any loss, theft or damage to personal property brought to the school.
  5. Media release: I grant the school permission to use photographs, videos or other media taken during school activities for promotional purposes, including the website, social media and brochures.
